Below are links to two different file formats of our Intake Forms. PDF format forms must be downloaded and printed, filled it out by hand, then mailed to us (you will need a PDF viewing application, like Adobe’s free Acrobat Reader below, to view and print this document).
Alternatively, you may download the Microsoft Word version of an Intake Form, fill it out using MS Word application, then e-mail us the completed form.
